MiHistorIA: Patient-centered unified medical record

Data simulated for demo use only

What we built: Digital health focused on the patient, not institutions

Today, medical information is fragmented: test results in PDFs, prescriptions in different portals, lab reports elsewhere, and doctor notes in emails or WhatsApp.

Mi HistorIA was designed for a Top-Tier Health Insurance Provider in Argentina around a simple idea: even if healthcare systems cannot integrate, patients can unify their own data.

The challenge: Fragmented clinical data and incomplete medical decisions

Patients face:

  • Scattered medical records.

  • Non-interoperable systems.

  • Lost physical documents.

  • Incomplete information during emergencies.

From routine checkups to arriving unconscious at an emergency room, fragmentation directly affects the quality of care.

The opportunity was clear: empower patients as the true owners of their medical information.

A unified medical record app built by the patient

Mi HistorIA enables users to:

  • Upload lab results and studies.

  • Store prescriptions and diagnoses.

  • Centralize medical instructions.

  • Build a structured health timeline.

  • It does not provide diagnoses, but it enables healthcare professionals to add medical recommendations, test results, and clinical interpretations directly within the app

Instead of relying on institutional integrations, the model shifts the center of gravity to the patient.

Impact and evolution: enabling smarter patient-driven healthcare

The initiative unlocks:

  • Better traceability of medical history.

  • Greater patient autonomy.

  • More informed care in critical situations.

  • A foundation for future intelligent features such as alerts and proactive monitoring.

The natural evolution toward an MVP would validate adoption and expand its role within the broader digital health ecosystem.

Mi HistorIA redefines ownership: medical history becomes truly personal.
